Advanced Practice Registered Nurse (Clinical Contact Center)

The Advanced Practice Registered Nurse (APRN) incumbent executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care in an episodic, urgent care setting. Care will be provided in the least restrictive, most cost-effective manner through use of the clinical practice guidelines and clinical pathways. Duties include, but not limited to:
Provides direct patient care and works with members of the expanded health care team, including:
Physicians, Psychologists, Social workers, Clinical Pharmacists, Nurses, Clinical Associates, Medical Support Assistants and other members of the Clinical Contact Center, in order to manage the urgent and episodic needs of patients, and coordinate medical care for a population of Veterans in the Clinical Contact Center practice setting. Provides care, emphasizing evidence-based medicine, primary and secondary preventative medicine, the use of a computerized medical record system, and continuous quality improvement based on individualized provider data. Formulates appropriate diagnosis based on history and physical evaluation and clinical findings. Monitors clinical contact center practice through tools such as PACT Compass, ED metrics, SHEP Scores, disease registries, and peer review. Participates in development, implementation, and evaluation of policies, procedures, and guidelines to improve the quality and function of the Clinical Resource Hub Serves as a resource and advocate in resolving legal/ethical issues such as Advanced Directives and the protection of patient/family rights. Prescribe medications and order diagnostic tests in a cost-effective manner. Performs GEMS responsibilities as defined in the VAMC Memorandum Green Environmental Management System Policy and shall consider environmentally preferable and recycled products in all purchasing decisions. Routinely evaluates, interprets, and ensure compliance of telehealth program and services based on established professional, community, or regulatory standards (i.e. The Joint Commission) and recommends and/or implements changes to meet or exceed applicable standards. Work Schedule:
Variable shifts includes covering evening and overnight tours for after-hours call center. May do 10 or 12 hour shifts. Coverage includes weekends and holidays and some rotation of shifts may occur. NOTE:
As we hire more staff, the shift will include day shifts as staff rotate. Recruitment/Relocation Incentive:

RN's appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Education. The Advanced Practice Registered Nurse (APRN) is a graduate of an NLNAC/CCNE accredited school of Nursing and meets VA professional and physical requirements as set forth in MP-5, Part 2, Chapter 2, Change 2 and VA Directive 5102.1 dated November 10, 1999. Must possess a Masters or Doctorate degree from an academic program accredited by the NLNAC or CCNE Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Certification. full and current certification as an APRN from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. (adult primary care, emergency care, urgent care or related field) Meet physical and professional requirements as set forth in MP-5, Part II, Chapter 2, Appendix F; VA Directive 5102.1 (current edition) and VA handbook 5005/27,Part II, Appendix G6 dated March 17, 2009. Experience:
3 years of healthcare experience in adult primary care, emergency care, urgent care or related field, with at least one year of Telehealth healthcare experience. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse II - Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires occasional periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on Veteran needs. Position may require occasional travel to VISN 8 medical centers and VISN 8 network office.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
