Summary:
Provides quality psychosocial care to our patient's and coordinate care with medical team.
Through the coordination of care, the primary purpose is to conduct a thorough assessment of patients' needs, understanding of the medical plan of care, coping abilities, and barriers to care then implement interventions and/or provide resources.
Serves as an integral member of the health care team delivering psychosocial care in accordance with JHACH policies and procedures and state and federal regulations.
PER DIEM, ROTATING SHIFT, WEEKENDS
Qualifications:
Master's Degree in Social Work Completion of JHACH's intern program to meet the qualifications of the Social Worker career requirements OR have successfully completed a comparable intern program in an external healthcare environment Excellent listening and oral/written communication skills Ability to document effectively in the electronic medical record Recommended Skills Communication Social Work Health Care Electronic Medical Record Psychosocial Estimated Salary: $20 to $28 per hour based on qualifications.
